Patients who are new to Colorado Center for Digestive Disorders and Colorado Endoscopy Centers will require some information that is provided on this page. A map and directions to our office is available please click Maps . To save you time, we have also included new patient forms as well as billing information, etc. In order to provide you with the most convenient service, please fill these forms out prior to the office visit. In addition, please send these to our office via email or via fax in order to expedite your office visit when you arrive.

What to bring to your office visit:

  • Complete the new patient office visit paperwork.
  • If this is a new medical problem not previously addressed through the office, please fill out pertinent information.
  • Obtain all previous records and diagnostic evaluation for your current medical problems. This includes previous colonoscopy, pathology reports. If you have been hospitalized recently, provision of that information is extremely useful in order to coordinate care.
  • A list of all physicians and their contacts.

What To Expect

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this website.
