It is important to bring the following information with you to your initial appointment:

	Current insurance card.
	Current drivers license.
	Current list of medications including doses and frequency.
	List of allergies.
	List of past and current medical problems.
	Any lab reports or information which has been provided to you by your current or previous physicians.

By completing the questionnaire, you will decrease the time required prior to your office visit. All information given to us is sent through a secure server. It is recommended that you fill out this form after you submit an appointment request; however, if you have already provided this information to Colorado Center for Digestive Disorders, you do not need to do so again.
